This is not a Notice of Privacy Practices
Omni's approved, entity-specific Notice of Privacy Practices has not yet been supplied for publication on this website. This page is general guidance only; it is not that notice, a substitute for it, or a statement that this website is HIPAA compliant.
A healthcare provider subject to HIPAA generally must make its current Notice of Privacy Practices available, including prominently on its service-information website. Before clinical use or public launch, the responsible provider must confirm its legal entity, covered-entity status, privacy contact, current practices, applicable record protections, and approved notice.
Request the treating provider's notice
Contact NBJPN Holdings by phone and ask which legal entity will provide your care and how to obtain that provider's current Notice of Privacy Practices. A clinical provider's notice should explain uses and disclosures of protected health information, patient rights, provider duties, how to make requests or complaints, and who to contact.
Do not use this form for records requests that include health information. Ask the provider for an approved records-request process and secure communication method. General website acknowledgement is not acknowledgement of receipt of a clinical privacy notice or authorization to disclose medical records.
General privacy rights
Where HIPAA applies, rights may include requesting access to or a copy of records, requesting corrections, requesting confidential communications or certain restrictions, receiving an accounting of certain disclosures, and obtaining a copy of the provider's notice. Rights and exceptions depend on law and the relevant records.
Certain records, including some substance use disorder treatment records, may have additional protections. The treating provider must explain applicable requirements. This website does not establish which specialized rules apply to Omni's services.
Concerns and complaints
You may ask the treating provider about its privacy complaint process. You can also learn how to file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. HIPAA prohibits retaliation for exercising protected rights or filing a complaint.
